Greetings Troop 101~
It is that time of year again! It is time to get your Health Forms submitted for Summer Camp. Here is the link to the Health form:

Please remember the following:
- EVERYONE who is planning to attend summer camp must submit a Health Form & copy of insurance card. Make a copy for yourself as well.
2 Section C of the Health Form requires a signature from your child’s MD - You are also required to submit a copy (front and back) of your insurance card with the Health Form.
- I would like to have your Health Form by May 15th as I have to submit them to the camp prior to our arrival in June.
- If your child turns in a signed and fully completed Health form along with a copy of your insurance card on their first attempt, I will reward them with a Candy Bar!
- Please turn your health form into me during announcements at the Monday Troop Meetings or put it in the water-proof envelope under my front porch mat at 1507 Persimmon Place.
- If you are not planning to attend summer camp, you must submit part A & B as well as a copy of your insurance card in order to attend troop camp-outs.
