Troop 101 Adults/Scouts:
It is a new year and that means a new BSA Health form needs to be completed and turned in to the Troop.
EVERY REGISTERED ADULT AND SCOUT IN THE TROOP, MUST COMPLETE A NEW BSA HEALTH FORM EVERY YEAR!
There is a new version of the BSA Health form to complete. It is available on TroopTrack under Share-Troop Documents. I have also attached a copy of the form to this message.
Parts A, B1, and B2 are required for everyone to complete. If you are planning on attending Summer Camp, you must also complete Part C – the Pre-Participation Physicial. This section must be completed and signed by a physician.
When turning in the health form, please be sure to attach a front/back copy of your insurance card.
Once forms are completed, they may be turned in at our weekly meetings to Mrs. Rinker.
