Troop 101 has created a Summer Camp Medication Chart to be filled out and turned in at check-in on departure morning. This form is to help insure that all medications intended to be taken at camp, arrives at camp. I have attached the form below.
This form is in addition to the note card the must also be provided with the dosage information for the medication. THIS FORM MUST BE COMPLETED BY ANY PERSON ATTENDING SUMMER CAMP THAT IS BRINGING MEDICATION. Please print the form to turn in on departure morning.
At check-in on departure morning, you will check in with Mrs. Baker and then check in with Mrs. Rinker to turn this medication chart.
